All of our members have access to the highest-quality care from medical specialists in every area of health for seamless and personalized care at a PACE center, their home, or at an inpatient facility.

We offer support to caregivers at our Adult Day Care. Here, caregivers can drop off their older adult family member or friend who does not require 24-hour care while they are at work or for a day of rest. As a caregiver, you can be confident that your loved one is receiving top-notch care that feels like home with social activities, great meals and snacks, and consistent health management.

We are always here when you need us. Our comprehensive emergency and urgent care services are a part of our all-inclusive care model and ensure that participants receive immediate attention when unexpected health issues arise. Whether it’s a sudden illness, a minor injury, or something more serious, our team is prepared to respond quickly and effectively.

WellQuest PACE provides safe, door-to-door transportation for all participants so that they can get to medical appointments, therapy sessions and wellness visits, to the Day Health Center for activities and meals, and hospital or urgent care trips when necessary. Our trained drivers help participants in and out of vehicles and maintain secure and comfortable traveling. Every ride is coordinated by the WellQuest PACE care team to make sure participants are never left waiting or wondering where to go.

We understand that caring for an aging loved one is both rewarding and challenging. That’s why we offer respite care and comprehensive caregiver support to give family caregivers the time, space, and peace of mind they need to rest and recharge to prevent burnout. Not only do we offer our Day Health Center to provide care for their loved ones, but we also offer emotional and educational support and care coordination so they can rest assured that their loved one has everything they need.

Our licensed social workers and support staff play a vital role in caring for the whole person, helping participants and their families access the resources they need to live safely, independently, and with dignity. We create a personalized plan of care that addresses the social, emotional, and practical needs of each member by working closely with medical providers, therapists, and families.

WellQuest PACE offers access to a wide range of lab and diagnostic services to help monitor health, catch concerns early, and guide personalized treatment plans. From blood work and routine lab tests to specific diagnostic screenings, we prioritize keeping you informed with quick results and coordinated follow-up care.

Our licensed therapists work closely with members and their care teams to create personalized therapy plans that focus on restoring function, building confidence, and supporting daily living skills.

We make sure members get the medications they need, safely and conveniently. Our on-site medical team works closely with each participant, so there’s no need to worry about insurance paperwork, pharmacy trips, or unexpected costs.

For older adults who prefer to receive care in the comfort of their home, we will bring skilled medical and personal care right to your door. Our experienced home health care team provides a variety of services, including medication management, health monitoring, therapies, personal care assistance, medical equipment set up, and more.

Our health care team focuses on preventative care to help participants maintain their health, avoid illness, and catch potential problems early, before they become serious. We offer a variety of services, including regular health screenings and check-ups, vaccinations and immunizations, health and wellness counseling, and nutrition and exercise guidance.

We believe that true care should meet each of our members’ unique needs so that they can live comfortably and confidently in their own homes and communities. Our trained caregivers assist with everyday activities such as bathing and grooming, dressing, toileting, transportation, meal preparation and feeding assistance, medication management, and light housekeeping.

While WellQuest PACE is designed to help participants stay healthy and independent at home, we understand that there may be times when hospital or nursing home care is necessary. When those situations arise, we are here to coordinate and cover the care you need. Our team can provide coordinated hospital admissions and discharge planning, skilled nursing care in short- or long-term care facilities, personalized recovery plans, and transportation.

We know that good nutrition is a key part of staying healthy, active, and independent. That’s why we provide nutritious meals and personalized nutritional counseling to support the unique dietary needs of each member. Our team will come up with customized meal plans, provide one-on-one nutritional counseling with a dietician, serve healthy meals at the Day Health Center daily, and offer assistance with grocery and meal planning.
